COVID-19 Questionniare

The COVID-19 Questionnaire is a brief list of yes or no questions that were selected by your institution to help maintain the safety of staff and students. When you log in to your CourseKey account each day you'll be prompted to complete the questionnaire, and based on your responses you'll be provided with a digital green or red card. 

 

Logging in on Chrome

You can access your CourseKey account using the Chrome web browser on a computer, tablet or mobile device. On your first log in each day you'll see a prompt with the following question:  "Will you be visiting a physical site in association with your program today?"

No, I do not plan to visit a site.

If you will not be visiting a physical location that day you can choose to bypass the form by selecting "No" and continue using CourseKey as needed. 

Yes I plan to visit a site.

Select "Yes" if you plan on visiting any physical location that day. The questionnaire will be displayed with a series of yes or no questions. 

If you choose to complete the form each question must have yes or no marked on the available radio buttons to proceed. Select the check boxes at the bottom of the form if you agree to the terms and conditions and certify that your responses are true and accurate to the best of your knowledge. Select Submit to confirm your responses.

 

Green Pass

If you are cleared to visit a physical location you'll receive a Green Pass for the day. This pass can be accessed at anytime throughout the day from within your account, and it will expire at midnight.

 

Red Card

After you submit your responses you'll be issued a red card if one or more of your responses requires follow up with your COVID-19 site administrator. A button will be available to open an email to communicate with the designated administrator. Please contact them at your earliest convenience, and do not go to the site.

 

 

Open Your Card / Return to Form

After completing the questionnaire your green or red card will be stored within your account. This can be accessed at any time throughout the day, and it will expire at midnight. Follow steps 1 & 2 listed below to access your card.

What if I accidentally said 'No'?

If you accidentally said 'No you will not be visiting a site' or if you had a change of plans, you still have the option to complete the form. By selecting Health Check from your profile options you'll see the question again "Will you be visiting a physical site in association with your program today?".  Select Yes to access the form.

Follow these steps to open your card or return to the COVID-19 Questionnaire :  

1. Within your CourseKey account select the avatar icon in the top right corner.

2. Select Health Check from the drop-down menu.

 

 

CourseKey Student Mobile App

Downloading the CourseKey Student mobile app can give you quick and easy access to the COVID-19 Questionnaire.

When to Use the Mobile App

Although you'll use the same username and password, it's important to note that as an instructor you will not be able to access any course information through the mobile app. Any of your CourseKey activities that are unrelated to the COVID-19 Questionnaire must be accessed in the CourseKey Portal using the Chrome browser.

Accessing the COVID-19 Questionnaire in the Mobile App

When you first log in to the mobile app you'll receive the same prompt that is displayed on the web "Will you be visiting a physical site in association with your program today?" and you'll continue to complete the questionnaire if needed. 

After completing the questionnaire your green or red card will be stored within your account profile. Follow these steps to open your card or return to the COVID-19 Questionnaire form:  

1. Within the CourseKey app select the avatar icon to view your account/profile.

2. Select Student Health Check under account details.
